Guidelines for Hemodialysis Registry

Register your center by clicking on the “Sign Up” button.

Complete the form and submit.

Once your form is approved by the Administrator, you will receive an acknowledgement email.

“Sign In” with your email address as the user name and your chosen password.

Click the “New Patient” tab and complete the data for each patient..

Fill out either the “Date of Birth” or “Age” of the patient in the data form. On filling out one of fields the other field will automatically be completed.

Similarly either fill out the “Date of 1st Dialysis” or the “Months on Dialysis” field on the form. On filling out one field, the other field will be completed automatically.

The patient’s CNIC number and phone number is considered essential data as it the primary source of patient identification and helps avoid any duplication.

Laboratory data to be entered in the registry must be predialysis (predialysis labs with one day interval in case of thrice a week dialysis).

Although there are a few essential fields, it is imperative that every attempt is made to fill out all the fields so ensure completion of registry and richness of data for research purposes.

All patient and center data will be updated on a six monthly basis.

In case of a significant change in any patient’s status (e.g. change in hepatitis serology status, change in vascular access, death, transplantation etc.) the patient’s data needs to be updated immediately.
